Missing contact information form after upgrading in Dynamics 365

On the Magnetism Solutions Dynamics CRM Blog, Miguel Nepomuceno stated recently that Magnetism has encountered a lot of strange issues after customers upgrade their D365 instances to a new version.

The main problem is that customers are reporting missing components in their systems. As such, they are left to wonder where certain fields are in the form, specifically those dealing with the contact entity, he stated.

After further investigation, I have been getting same issue from different customers regarding one specific component – the contact Information Form always goes missing after an upgrade.

The contact information form is an out-of-the-box system form, said Nepomuceno. He added that most of Magnetism's customers use this form when they're managing their contact records. This out-of-the-box form frequently contains custom fields and other custom logic, he stated.
